Providence Community Living Services provide a multitude of support and services for people living in a community-based setting.

Wherever it is you call home, and whomever it is that you share it with, is a big part of who you are. Home is such an important place, so where we call home should always be our personal choice. Providence is here to support you in exploring your options for living as you find the right place to call home.

Community Living Includes

Community Living – Group Home (Charles County)

This service provides people with support for the development, acquisition, and maintenance of skills related to activities of daily living, instrumental activities of daily living and socialization.

Services include coordination, training, mentoring, support and supervision as determined by an assessment of needs and as indicated in the  person driven plan.

Services are provided in a provider owned or operated residential setting, commonly referred to as a “group home.”

Supported Living Services

Supported Living Services provide people with a variety of services to support living independently in the community. These services are individualized to the person’s needs and interests as documented in their person driven plan and must be delivered in a personalized manner.

 

Supported Living Services assist people to:
  • Learn self-direction and problem-solving related to performing and maintaining activities of daily living required for the participant to live independently
  • Engage in community-based activities of their choosing within their personal resources.

 

Supported Living Services include assistance and facilitation with finding an apartment or home, roommates, and shared supports based on the person’s preferences and choice, and oversight of service quality and compliance with program requirements once the arrangement is established. These services enable people to live in a residence of their choice located where they want to live; and live with other people of their own choosing. Supported Living Services consist of activities of daily living such as: light cooking, cleaning, laundry, managing finances, doctor’s appointments, medication, community interaction and socialization. Depending on the person’s needs, preference, and person driven plan, support can be provided from 6-24 hours per day.

Overnight Supervision

Overnight services, commonly referred to as “awake overnight” are supports provided to a person during the night hours. This service is rendered according to the person’s assessed medical and behavioral needs and person driven plan.

Dedicated Support Hours

Dedicated support hours are services that provide support for people who are living in a residential setting. The support hours are based on the person’s assessed needs (medical, behavioral, community) and person driven plan.
